An FBI agent falls asleep during a stakeout of a religious cult. His partner wakes him. They check the building and find the bodies of 20 cult members, all killed by an axe blow to the head.
The murder kills both agents inside the building. When Skinner and Doggett arrive (Scully is seeing a doctor in hospital), one agent's body is found in his car, the other's is in his condo' - miles away.
The cult's leader, Anthony Tipet, is missing. Skinner's explanation, of Tipet's spirit (with a third eye) being the killer, is not taken well by the Deputy Director.
Skinner and Doggett question and arrest the dealer who provided Tipet's drugs. Doggett has a dream of the spirit just before it appears along with a hoard of rats, which bite the dealer to death in his cell.
Scully, who is still in hospital, asks the Lone Gunmen to help Doggett. They suggest to him that Tipet has succeeded in a mind control technique, previously the subject of CIA experiments.
As Skinner and Doggett find Tipet, he cuts his forehead open with a circular saw to stop the attacks. Tipet is in a comma and the hunt is called off. Doggett dreams that he sees the spirit in his house and then a reflection of himself in a mirror with a third eye.
The dream continues as he almost kills Scully, then himself with an axe. Tipet dies in hospital. Scully, who is now out of hospital - no explanation given, tells him it was just a bad dream.
