Joyce is in hospital, Buffy and Dawn are keeping her company. Her operation is in 2 days. Giles, Willow and Xander are patrolling.
A mental patient at the hospital questions Dawn and says that there is "no one in there". A meteor crashes into Sunnydale - a demon comes out of it, kills the mental patient seen earlier and proceeds to the hospital. Joyce is anxious to leave the hospital rather than waiting there for the operation.
The rest of the gang investigate the meteor and discover that it is hollow. They find the dead man with something in his mouth. While the gang go off to do some research, Riley calls the government for help.
Joyce leaves the hospital. The commando team arrives and, together with Riley, search for the demon. Joyce sees that Dawn is not what she appears to be and tells her. Three people have now said this to Dawn and she is upset.
The gang's research finds that the demon arrives on earth to kill people who are crazy. The demon is at Buffy's house and Joyce is it's next victim. It attacks her, Dawn attacks it and saves Joyce. Buffy kills the demon with some help from Spike (who was stealing photos of Buffy from the basement) just before the commandos and Riley arrive.
It turns out that the friendly hospital orderly, Ben, summoned the demon and knows Glory.
